Welcome to Bellava Aesthetics & Wellness. We are your trusted partner in navigating the Kentucky Medical Cannabis Program. In a remarkable effort to help Kentuckians move away from the dangers of addictive opioids and find relief from severe and chronic pain, Governor Andy Beshear signed Senate Bill 47 on March 31, 2023. This landmark legislation paves the way for the legalization of medical cannabis in the commonwealth, set to take effect on January 1, 2025. Together, we are embracing a compassionate approach to healthcare and enhancing the quality of life for our communities.
Bellava Aesthetics & Wellness is one of the very few providers in Kentucky that is a registered and certified provider to conduct medical evaluations to provide certifications for medical cannabis. To obtain your Kentucky medical cannabis card, an initial in-person evaluation is necessary to establish a genuine patient-practitioner relationship. We encourage patients to upload documentation of any pre-existing medical conditions through your secure portal that you will have access to once you book an appointment.
The fee for a medical cannabis certification is $150 due at the time of your service. We do not accept Insurance
Additionally, we offer the option for new PTSD diagnoses to be made during your visit for an additional fee of $150, though we cannot guarantee qualification.

Qualifying for a Medical Cannabis Card in Kentucky
Here’s how you can qualify:
Residency: You must be a resident of Kentucky.
Criminal History:
Ensure you have not been convicted of any disqualifying felony offenses.
Qualifying Medical Conditions:
You must have one (1) of the following qualifying conditions:
- Any type or stage of cancer
- Chronic, severe, intractable, or debilitating pain
- Epilepsy or other intractable seizure disorders
- Multiple sclerosis, muscle spasms, or spasticity
- Chronic nausea or cyclical vomiting syndrome unresponsive to conventional treatments
- Post-traumatic stress disorder (PTSD) If you meet these criteria, you’re on your way to accessing the relief that medical cannabis can provide!
