Certified Medical Cannabis Evaluations in Kentucky

Bellava Aesthetics & Wellness is proud to be among the very few registered and certified providers in Kentucky authorized to conduct medical evaluations and issue medical cannabis certifications.

To obtain your Kentucky medical cannabis card, an initial in‑person evaluation is required to establish a genuine patient‑practitioner relationship.

Certification Fees

How to Qualify for a Kentucky Medical Cannabis Card

1. Residency Requirement

You must be a resident of Kentucky.

2. Criminal History

You must not have been convicted of any disqualifying felony offenses.

3. Qualifying Medical Conditions

You must have one or more of the following qualifying conditions as outlined by Kentucky law :

  • Any type or stage of cancer

  • Chronic, severe, intractable, or debilitating pain

  • Epilepsy or other intractable seizure disorders

  • Multiple sclerosis, muscle spasms, or spasticity

  • Chronic nausea or cyclical vomiting syndrome unresponsive to conventional treatments

  • Post‑traumatic stress disorder (PTSD)

If you meet these criteria, you are well on your way to accessing the relief that medical cannabis can provide.

Bellava Aesthetics & Wellness is one of the very few providers in Kentucky that is a registered and certified provider to conduct medical evaluations to provide certifications for medical cannabis.  To obtain your Kentucky medical cannabis card, an initial in-person evaluation is necessary to establish a genuine patient-practitioner relationship. We encourage patients to upload documentation of any pre-existing medical conditions through your secure portal that you will have access to once you book an appointment. 
 
The fee for a medical cannabis certification is $150 due at the time of your service. We do not accept Insurance
Additionally, we offer the option for new PTSD diagnoses to be made during your visit for an additional fee of $150, though we cannot guarantee qualification.
